 
															Root Cause Analysis
All regulations for safety and compliance calls for root cause investigations to be performed following accidents/incidents, findings, or in response to reported problems.
Training in performing effective root cause analyses is relevant for safety managers, compliance monitoring managers, supervisors, auditors, and other staff involved in answering or evaluating audit findings and safety reports.
It is important that the analysis does not primarily focus on establishing who or what caused the non-compliance, but on why it was caused. Establishing the root cause(s) of a non-compliance often requires an overarching view of the events and circumstances that led to it – in order to properly identify all the possible systemic and contributing factors (regulatory factors, human factors, organisational factors, technical factors, etc.) in addition to any direct factors.
A narrow focus on single events or failures may not properly reflect the complexity of the issue.
Our Root Cause Analysis training offers various methods for analysing events. In addition, the training presents several tools to perform root causes – and case studies will be used to try the tools in practice. The training also discusses how to effectively establish corrective and preventive measures.
Essentially, an effective process for identifying and addressing the (proper) root cause will prevent recurring problems and save both time and money in the long run.
